- style number(s), color(s), and size(s) of the shoe(s) you wish to purchase
- your name, telephone number and e-mail address
- credit card number with expiration date
- shipping address (UPS will not ship to a PO box)

Do you know what you want and are in rush or don't have access to a computer? Fax the information to 605-229-5243. We need the following information to complete a fax order:
- style number(s), color(s), and size(s) of the shoe(s) you wish to purchase
- your name, telephone number and e-mail address
- credit card number with expiration date
- shipping address (UPS will not ship to a PO box)
We will contact you via e-mail or phone to confirm your order or pass along any other information.
